You'll collaborate with families, rehabilitation professionals, and the Veterans Affairs Canada Case Manager to devise effective strategies. Your expertise will be key in identifying barriers and implementing solutions for improved outcomes.

Key Responsibilities:
• Conduct rehabilitation assessments to determine participant needs
• Create tailored Rehabilitation Plans in collaboration with stakeholders




• Serve as the service integrator with rehabilitation professionals
• Facilitate community-based interventions for optimal wellness
• Develop strategies to overcome identified barriers

Requirements:
• Bachelor's degree in Psychology, Rehabilitation, or related
• At least three years in direct rehabilitation service
• Experience with individuals facing health-related barriers
• Obtain federal security clearance – Reliability Status required
• Ability to travel regionally as needed
