Take on a vital role as Clinical Care Manager – LPN, where clinical leadership meets operational support. This full time position focuses on promoting resident-centered care and staff guidance.

As the Clinical Care Manager, you will collaborate closely with the Director of Care. Your seasoned experience as an LPN will guide nursing staff through complex clinical needs and quality improvement initiatives, reinforcing communication and care standards. This role emphasizes the importance of teamwork and the significant impact on the resident and family experience.

Key Responsibilities:
• Provide leadership and support to nursing and care staff
• Assist with clinical rounds and care conferences
• Address resident and family concerns promptly




• Participate in quality improvement projects and audits
• Mentor staff on best practices and documentation standards

Requirements:
• Registered LPN in good standing
• At least five years in nursing, preferably in long-term care
• Leadership or supervisory experience is a plus
• Valid CPR certification
• Knowledge of electronic health records systems

Foster a culture of commitment and quality improvement within the nursing department as an LPN Clinical Care Manager.
