The Mustard Seed seeks a compassionate Social Worker for its Pathways to Reintegration Program in Edmonton, focused on mental health and addiction support. Help individuals transition back into the community successfully.

As a vital member of the team, you will provide assessments, counseling, and referrals to those experiencing homelessness and substance use issues. Your leadership will empower clients to navigate recovery pathways while collaborating with other community resources. This role is critical in supporting clients’ mental health needs for a successful reintegration.

Key Responsibilities:
• Assess clients’ mental health and substance use requirements
• Provide direct counseling and client care
• Conduct group interventions addressing substance use disorders




• Maintain accurate documentation of client progress
• Collaborate with multidisciplinary teams for case management

Requirements:
• Bachelor’s Degree in Social Work
• Minimum two years in Mental Health services preferred
• Knowledge of Edmonton’s community resources
• Ability to communicate effectively with diverse clients
• Clean driving record and valid License required
