Join Mi’kmaw Native Friendship Centre as a Supportive Housing Intensive Case Manager in Halifax, NS. In this full-time role, you'll assist Indigenous families in stabilizing their housing situations amidst complex needs.
The Supportive Housing Case Manager will play a crucial role within the MNFC's housing team, focusing on the unique needs of Indigenous service users. Utilizing a decolonized, trauma-informed lens, you’ll support families through personalized case plans while addressing factors such as substance use and mental health. Your advocacy will enhance their access to vital resources, contributing to improved housing stability and a better quality of life.
Key Responsibilities:
• Conduct intake assessments and build partnerships with clients
• Implement strength-based case plans to achieve client goals
• Facilitate access to culturally relevant programming and resources
• Document client interactions and maintain case records
• Attend lease signings and support housing application processes
Requirements:
• Diploma or certification in community social services
• Minimum two years in social services or related work
• Knowledge of Housing First and harm reduction principles
• Solid interpersonal and written communication skills
• Valid driver's license required for client transportation
