As a full time Geriatric Nurse Coordinator, you will provide advanced nursing services within an outpatient geriatrics program. This role requires expertise in clinical triage, care coordination, and integrating geriatric assessments to support optimal patient outcomes. You'll collaborate with an interdisciplinary team to ensure comprehensive patient and caregiver-centered care while striving for service excellence.

Key Responsibilities:
• Review complex referrals using advanced clinical judgment
• Conduct thorough comprehensive nursing assessments




• Coordinate nursing and clinical support for scheduled clinics
• Anticipate patient care requirements to optimize clinic flow

Requirements:
• Bachelor's Degree in Nursing
• Current registration with College of Nurses of Ontario
• Minimum three years of geriatric clinical experience
• At least two years in complex care coordination
• Completed training in Geriatrics or commitment to complete
