Join CMHA-YRSS as a Registered Practical Nurse with our Assertive Community Treatment Team in Vaughan. This role focuses on mental health support for individuals, ensuring they receive tailored recovery assistance.

As a vital member of the South West program, you will leverage your skills to conduct assessments and create treatment plans for clients facing significant challenges. This full-time position involves both in-office and work from home, providing flexibility in how you deliver care.

Key Responsibilities:
• Develop and implement comprehensive treatment plans for clients
• Conduct thorough mental health assessments and referrals
• Establish client partnerships for independent living
• Monitor progress and connect clients with community support




• Provide short-term assistance and crisis intervention

Requirements:
• Licensed as a Registered Practical Nurse, Ontario College of Nurses
• Four+ years of experience in a mental health setting
• Strong diagnostic skills and clinical judgment
• Familiarity with the Ontario Mental Health Act
• A second language is an asset, valid driver’s license required
