Contribute to mental health improvement in the community as a Licensed Practical Nurse Outreach with Fraser Health. This position focuses on patient assessments, care planning, and delivering nursing services at home.

As an LPN Outreach, you will play a key role within Fraser Health's multidisciplinary team, working alongside RNs and RPNs to safeguard and enhance the wellness of clients with mental health issues. Your duties will include documenting care activities, assessing clients, and developing care plans based on individual needs and experiences. Engage in quality improvement and assist in teaching new staff members.

Key Responsibilities:
• Administer medications and monitor patient responses
• Conduct thorough assessments of client's needs




• Create and update comprehensive care plans
• Assist clients with daily living exercises
• Ensure safety standards are maintained in all care settings

Requirements:
• Graduation from an approved Practical Nursing program
• Active registration with BCCNM
• Current First Aid, CPR, and Food Safe certificates
• Valid driver's license and access to a vehicle
• Robust communication and organization skills
