VHA Home HealthCare seeks a Registered Nurse to join our dedicated Palliative Care team. This position involves delivering comprehensive end-of-life care and working alongside external health professionals. You will manage your schedule, evaluate care plans, and ensure quality nursing practices in a flexible work workplace.

Key Responsibilities:
• Assess client needs and coordinate care plans
• Monitor and evaluate ongoing care interventions
• Maintain high standards in patient care delivery
• Document client interactions and changes accurately
• Participate in on-call rotations for urgent care support

Requirements:
• Registered with the College of Nurses of Ontario
• RN diploma or degree from an accredited institution
• First Aid/CPR certification required
• Proven communication and documentation skills
• Valid Ontario driver’s license and reliable vehicle
