In this role, you will work collaboratively with the Care Designer and care team to develop and update care plans based on assessments. You’ll administer medications, monitor outcomes, and provide essential health education to promote independence. Your contributions will ensure clients receive safe and consistent care while addressing any changes in their conditions.

Key Responsibilities:
• Deliver nursing care within the scope of practice
• Conduct assessments and support care planning
• Administer treatments and document outcomes
• Monitor and respond to client condition changes
• Support caregivers through guidance and communication

Requirements:
• Current RN registration in valuable standing
• CPR certification as required
• Completed Criminal Record Check
• Excellent verbal and written communication skills
• Ability to travel to client visits as necessary
