In this position, you will deliver client-centered care, assess health needs, and coordinate treatment plans for older adults. Key Responsibilities:
• Conduct extensive client health assessments
• Diagnose and treat chronic/acute conditions
• Collaborate on individualized care plans
• Maintain thorough documentation of clinical interactions
• Advise clients and families on health management strategies
Requirements:
• RN (Extended Class), certified in Ontario
• Completed Nurse Practitioner program
• 2-3 years relevant nursing experience
• Geriatric or transitional care experience advantageous
• Robust organizational and interpersonal skills
