This full time position involves planning and evaluating care, working closely with cross-functional healthcare teams. Key Responsibilities:
• Assess and implement nutritional care plans
• Monitor trends in residents' nutritional status
• Consult with healthcare staff on dietary needs




• Document interventions within resident care records
• Educate staff on nutrition topics bi-monthly
Requirements:
• Bachelor's degree in Food & Nutrition required
• Completion of a Dietetic Internship necessary
• Current registration with the local Dietitian body
• Experience in long-term care is advantageous
• Excellent communication and organizational skills
