Become a part time Registered Dietitian with Extendicare, focusing on nutrition and hydration care for residents. This role emphasizes personalized care and interdisciplinary teamwork.
As part of Extendicare, you will assess residents' nutritional needs and create tailored care plans. This entails ongoing evaluations and education on dietary needs for residents and their families, alongside participation in care teams and committees to further elevate care standards in our facilities. Collaboration with staff on nutrition programs is also key to your responsibilities.
Key Responsibilities:
• Assess and document resident nutrition at key intervals
• Develop individualized nutrition/dietary care plans
• Provide ongoing dietary counseling to residents and families
• Participate in care team meetings as possible
• Oversee quality improvement in nutrition services
Requirements:
• Accredited degree in Food and Nutrition required
• 1-2 years of experience in relevant fields
• Registration with a provincial Dietitian College
• Strong skills in communication and care coordination
• Familiarity with healthcare software needed
