In this position, you will be the primary contact for patients, ensuring they receive timely care throughout the CTEPH continuum. You will work closely with healthcare professionals to assess, manage, and educate patients about their condition. Your role is vital in facilitating diagnostic testing and supporting multidisciplinary case discussions for optimal treatment planning.

Key Responsibilities:
• Manage assessments and investigations of CTEPH patients
• Facilitate timely consultation appointments for referrals
• Oversee coordination of essential diagnostic tests




• Monitor patient pathways and provide emotional support
• Liaise with patients regarding their treatment and self-management

Requirements:
• Bachelor's degree in Nursing; Master’s preferred
• Current Certificate from the CNO
• Experience in respirology or cardiology preferred
• Strong decision-making and problem-solving skills
• Proven ability to multitask effectively
