As a vital member of the Primary Health Care team, you'll perform comprehensive health assessments and engage in program development. You will provide necessary treatments and health education while collaborating with a diverse inter-professional team. This role’s flexible schedule allows you to harmonize work with other life commitments, ideal for driven professionals.
Key Responsibilities:
• Conduct ongoing health assessments and create individualized care plans
• Collaborate on patient-centered program initiatives
• Advocate for clients and facilitate access to community resources
• Educate clients and families on care processes
• Maintain meticulous records of patient interactions
Requirements:
• BScN Degree and registered with relevant authorities
• 5+ years clinical experience in community health
• Experience in consultative roles for other healthcare providers
• Strong problem-solving and decision-making capabilities
• Valid Ontario driver’s licence and vehicle access
