What We Do

We are an integrated health system providing the full continuum of care to the six communities of the western James Bay coast with over 500 employees, physicians, and partners in care. As a health authority, you will work in a unique setting where you have the chance to care for patients in all phases of their life journey, in all areas of their health experience. From Primary, Acute, and Mental Health, Dental, Specialty Clinics, Population Health and the WAHA Paramedic Service, the opportunities for growth are boundless. We provide community‑based programming in all six of our communities, have six paramedic bases, three hospitals, four emergency departments, one community health centre and a total of 72 beds. Our guiding goal is to be a centre of excellence in Indigenous and remote healthcare.

Who You Are

- Identifying and managing discharge for population groups at high risk for readmission
- Establishing expected date of discharge upon admission




- Patient education about diagnoses, medications, and other therapies
- Patient and family involvement in the discharge planning process
- Discharge pathways for target populations and for specific destinations
- Discharge summaries and written material for patients and downstream service providers

What You’ll Do

- Promote a plan that helps individuals achieve their best possible level of functioning both at home and within the community
- Work closely with the healthcare team to create an effective discharge process and community care plan
- Negotiate special arrangements to facilitate the appropriate early discharge of patients, allowing them to recuperate as close to home as possible
- Collaborate with the healthcare team to plan effective discharge and community care processes to ensure a smooth transition back into the home environment
- Ensure that inpatients discharged from WAHA are prepared to function independently in their home community in the north
- Collaborate with various healthcare disciplines and community agencies, such as Community Home Care Services, Payukotayno Agency, Friendship Centre, Community Daily Living, local Public Health Offices, foster care homes, Community Care Access Centre, and Peetabeck Health Services, to provide optimal support and care at home and within the community




- Assess and facilitate the placement of seniors who require specialized care at home or admission to a healthcare facility for appropriate care (e.g., Long‑Term Care, Palliative Care)
- Involvement in relevant community committees to address the current needs of the population serviced
- Schedule outreach visits to the coastal communities to engage in networking activities and information sharing. Outreach to band health services, health and nursing stations, other relevant service providers to ensure relevance of community services

What You Bring

- Diploma in Nursing required
- Bachelor of Science in Nursing preferred
- Gerontology Certificate
- GPA, U‑First, BLS
- P.I.E.C.E.S., DementiAbility
- Positive Approach to Care
- Mental Health First Aid for Older Adults
- Valid Driver’s License
- Preference will be given to those candidates who are Cree from the Mushkegowuk Territory
- Ability to communicate in the local native Cree language is an asset

Salary

$87,457.50 – $94,828.50
