Reporting to the Youth Homes Supervisor/Manager, the Youth Outreach Worker plays a crucial role in providing personalized mentorship and crisis support. You will be responsible for developing case plans for youth and ensuring timely service delivery while collaborating with community organizations. Proficiency in Microsoft Office and an understanding of community resources are vital for success in this role.
Key Responsibilities:
• Support youth through case management and advocacy
• Ensure professionalism in client interactions
• Coordinate transportation for youth to activities
• Develop and implement individualized case plans
• Support youth in job searching and skill development
Requirements:
• Experience in human services and case management
• Strong organizational and time management capabilities
• Proficiency in Microsoft Office Suite
• Culturally responsive practice with Indigenous populations
• Valid Class 5 Saskatchewan Driver's Licence
