As an LPN at AgeCare, you will play a crucial role on the inter-professional team, reporting directly to the Director of Care. This full time position involves conducting care assessments, ensuring accurate documentation, and providing direct care aligned with provincial standards. Key Responsibilities:
• Conduct comprehensive resident clinical assessments
• Maintain accurate care/service plans and documentation
• Evaluate effectiveness and modify care plans as needed




• Support resident care conferences and participate when relevant
• Administer treatments and medications while ensuring compliance

Requirements:
• Current Practical Nurse Diploma and registration in good standing
• At least 3 years’ experience in senior health care
• CPR certification and proficiency in Microsoft Office
• Proven leadership and mentorship skills
