RESPONSIBILITIES & DUTIES:
The position is responsible for the following within the scope of a Registered Nurse (RN): Conducts comprehensive assessments and reassessments of the mental health care and basic needs of individuals referred to the Community Treatment Order (CTO) program.
Meet with referred clients to complete standardized assessment upon intake to the CTO program and reassessments according to agency standards.
Provide ongoing clinical assessments of the client’s mental health and risks and observes for changes in symptoms and behaviours which may indicate a deterioration in client’s mental health during face-to-face appointments on at least a weekly basis (or as directed by the CTO).
Provide support, advocacy and community referrals addressing the ongoing needs and goals of the client.
Uses a high level of nursing knowledge, critical thinking skills, sensitivity and good judgement when assessing clients and circumstances for the purpose of treatment and support.
Provides clinical intervention to support client needs.
Develops goal focused support/care plans based on clinical assessment and reassessment with client and other supports as appropriate and requested by client, including family members, substitute decision makers, public guardian/trustees, and/or other natural supports, service providers.
Provides clinical nursing assessments regarding medications, side effects and medical conditions to care providers where required within the circle of care.
Coordinate involvement of other agency staff and outside resources, working toward the best interest of the client as a member of the community support team.
Ensure client interventions address outcomes or goals as derived from the client’s plan of care.
Ensure each client has a crisis plan on file
Ensures objectives of program are met.
Provide personal psychiatric rehabilitation, counseling and therapeutic support to client in achieving goals, particularly in the areas of keeping all psychiatrist and injection appointments, basic life skills, social Interactions, family relationships,



coping with mental illness, medication education and compliance, vocational skills development, monitoring progress and carrying out interventions when problems or crises arise.
Participate in the evaluation of program goals and objectives making recommendations on changes to current program activities for improved service delivery.
Ensure direct and timely communication with the treating psychiatrist regarding changes in symptoms or in the development of behaviours which may signal deterioration in the client’s mental health.
Completion of Progress Summary Reports for CTO as required.
Develops educational programs relevant to client group.
Organize and conduct skill training, educational, or support groups for clients at varied locations as requested by the manager.
Obtaining and developing resources for delivery of services and evaluating the effectiveness of the activities and sessions.
Assist clients in accessing and obtaining other community resources where needed and appropriate. Advocate with and for clients to ensure adequate resources are available. Where resources do not exist or are inadequate, advocates within system to develop or improve essential services and resources for individuals with serious mental illness.
Ensuring clients have an awareness and understanding of their rights under the Mental Health Act and other related legislation.
Under authorization of a psychiatrist or physician, receives medication orders, administers medications, including injectable medications, where appropriate.
Adhering to standards as set out by the College of Nurses of Ontario and agency Policy and Procedures.




Following agency procedures and College of Nurses of Ontario Standards regarding documentation of medication orders, medication administration and reporting of errors.
Conducts ongoing clinical assessment and observation for effects and side effects to the prescribed medication.
Acting, with client’s consent, as liaison with client’s psychiatrist/physician
Effectively communicates clinical assessments and concerns regularly with team members and psychiatrists/physicians and collaborates to establish interventions and supports based on client needs and goals.
Assess needs and values of family where family involvement has been indicated in order to support, link, educate and advocate for family as needed.
Provides back-up and coverage support for the CTO Coordinator as required
EDUCATION & SKILLS:
Requires knowledge and skills acquired through completion of University degree. BScN in Nursing. Current registration with the College of Nurses of Ontario is required.
Two years recent experience working with individuals on a Community Treatment Order.
Comprehensive knowledge of mental illness and treatments; comprehensive knowledge and skill in the functions and principles of case management; up to date knowledge of relevant mental health policy and legislation including the Mental Health Act; knowledge of community resources; effective oral and written communication skills.
Computer literacy skills and working knowledge or various Microsoft Windows application is required.
Bilingualism in both official languages at the advanced level is considered an asset.
A valid Ontario Driver’s License with access to a reliable vehicle and proof of current insurance and licensing are required for this position.
Hours of Work: Thirty-Five (35) hour work week. The agency’s regular hours of operation are Monday to Friday, 8:30 AM to 4:30 PM. Flexible schedule are required to meet client and service needs.
Salary range : Grade 9, $37.84 – $44.81 per hour
