• Complete detailed client assessments and care plans
• Lead care for clients in your caseload
• Support clients with transitions to community services

Requirements:
• Registration with the College of Nurses of Ontario
• CPR certificate and clear criminal background check
• Passion for community care and excellence in practice
• Capacity for physical demands of the job
• Flexible availability for shifts, including weekends
