Provide essential care coordination with Ontario Health atHome, supporting patients in their home environments. This role involves patient assessments and crafting tailored care strategies in Bells Corners.

As a Care Coordinator, you will assess diverse patient needs, collaborating closely with caregivers and healthcare teams to enhance patient engagement and service accessibility. Your focus will be on developing effective care plans and coordinating services that address both clinical and psychosocial challenges for patients. Your contributions will significantly impact their home care experience.

Key Responsibilities:
• Perform detailed assessments of patient and caregiver needs
• Create customized care plans with patients and families
• Oversee delivery of home health services




• Ensure effective communication among healthcare providers
• Document cases in accordance with legislative policies

Requirements:
• Registered health skilled with Ontario certification
• Two years of relevant community experience preferred
• Mandatory COVID-19 vaccination proof
• Access to a vehicle with a valid driver’s license
• Bilingual capabilities in English and French are preferred
