Nova Scotia Health Authority is seeking a Health Care Social Worker (BSW) for Primary Health Care in the Cape Breton region. The position supports the PHC EZ Strat Access Stabil team with comprehensive psychosocial assessment, planning and care coordination.

The successful candidate will hold a BSW, be registered with the Nova Scotia College of Social Workers and bring post-BSW experience in adult community mental health. Collaboration with an interdisciplinary team is essential.
