The Coordinator will provide assessment, intervention, and evaluation services using social service work knowledge, skills and strategies to assist residents and their families in achieving optimal social functioning with the Home, including the provision of psychosocial and counselling supports as identified and needed. Working in collaboration with the Manager, Allied Health & Academic Liaison, nursing leadership team and members of the interprofessional team, the Social Service Worker & Admissions Coordinator will provide the services within the scope of practice and in alignment to the Vision, Mission, Values and Ethics of Providence Care.Skills and Qualifications Required:Social Service Worker DiplomaCurrent registration with Ontario College of Social Workers and Social Service WorkersExperience working with seniors, families and vulnerable populationsExperience working in a Long-Term Care Home setting, preferredUnderstanding of medical terminology,
