LPN / LVN - LTC

Shift: 07:00 AM - 07:00 PM

Description:

We are currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Long-Term Care (LTC) team.The LPN plays a critical role in providing direct nursing care and supporting the health and well-being of residents in a respectful, person-centered environment.Provide direct nursing care to residents, including administering medications, performing treatments, and monitoring health status.Conduct ongoing assessments and report changes in resident condition to the RN and physician as needed.Support the development and implementation of individualized care plans.Supervise and support personal support workers (PSWs) or care aides to ensure quality care delivery.Maintain accurate and timely documentation in compliance with facility policies and regulatory standards.Promote and maintain a protected, clean,



and comfortable environment for residents.Communicate effectively with residents, families, and the interdisciplinary care team.Participate in shift reports, care conferences, and quality improvement initiatives.Respond promptly to emergency situations, following established protocols.

Requirements:

Active Licensed Practical NurseCPR/BLS certification required.Previous experience in Long-Term Care, geriatrics, or a related healthcare setting preferred.Excellent clinical, assessment, and communication skills.Compassionate, patient, and respectful approach to resident care.Ability to work collaboratively in a team environment.Knowledge of infection control and safety procedures.
