Job Title: Clinical Coach

Department: Clinical

Job Type: Temporary Full time

Wage: $29.89 per hour

Duration: Approximately 8 months (to end March 2027)

The HNHB LTCH BSO Clinical Coach (BrantRegion) will use a team approach to provide holistic care to the residents ofLTCH’s to which they are referred. Their practice and clinical approach throughshall support the capacity building of LTCH staff; coaching, mentoring andmodeling strategies and interventions to support the LTCH to manage and carefor residents with responsive behaviours using best practices, externalsupports by utilizing their clinical standards of practice in accordance withtheir regulating body. Behavioural Supports Ontario aims to improve the qualityof care and quality of life for individuals who are at risk of, or areexperiencing, responsive behaviours and their caregivers, through theimplementation of the BSO Action Plan in the Hamilton, Niagara, Haldimand andBrant regions.

We are currently looking for a temporaryfull-time Clinical Coach(RPN)for the Brant Region until March 2027.

KEY ACCOUNTABILITIES & SUCCESS CRITERIA

- Workscollaboratively, professionally and ethically with other externalproviders/ outreach teams in that sub region or from a referring communityor sub region in the support of the LTCH which they provide services to.
- Worksalongside, not in place of staff in the LTCH in order to:
- Assess and observe the interactions ofthe resident with staff/ other residents and the environmen
- Obtain staff input on pastinterventions trialled and the application of new recommendations or strategies
- Assist with care and transfers (standby, demonstrate, coach, mentor and model)
- Determine other residents who may beprone to or at risk to display responsive behaviours for which a referral couldassist the resident and the staff
- Build relationships with LTCH staff andto coach best practices
- assess the effectiveness of strategiesin order to evaluate the readiness of a case to close
- support and demonstrate Montessori orbehaviour reduction activities at large with residents in collaboration withthe LTCH staff, Therapeutic Recreation department, Therapy Department andleadership.



(i.e. Montessori activities, reading circles, doll therapy,domestic activities such as dusting, sorting, towel folding, games etc.) todemonstrate their effectiveness as a staff capacity approach in the reductionof responsive behaviours, strength-based approaches, sun-downing interventions,and quality life for the residents.
- Contributesto or conducts approved resident assessments in the development of a holistic plan of care based on the needs of the resident and the referringissue including a PIECES assessment on all new referrals.
- Contributesto the resident’s care and services in consultation with the LTCHinterdisciplinary team, the resident, their family and external providersthrough analysis, data collection, observation, and trialling ofstrategies alongside front line staff in the LTCH home.
- Providesrecommendations and strategies in an attempt to reduce, prevent ordeescalate the responsive behaviours of the resident for which they arereferred to or asked to consult on by the contact person or leadership ofthe LTCH to which they are assigned or are covering.
- Ensuresthat the strategies they have recommended and / are trialling throughmentoring, coaching and modelling are regularly evaluated and revised asneeded and communicated to the home staff through face to faceinteraction, shift report, team meetings, Responsive Behaviour/ Riskmeetings and documentation.
- Opensand closes cases in accordance with established practices within BSO andin consultation with the LTCH Home.
- Usesthe training, assessment tools and resources provided including but notlimited to PIECES, Cornell and GDS depression scales, pain assessments,Cohen Mansfield's, CAM and in consultation or collaboration with thehomes.
- Providesmentorship and support that is person centred and based on best practices.
- Attendscase conferences,



sub region meetings and complex case meetings regardingtheir assigned or referred cases or LTCH’s where their schedule allows.
- Communicateseffectively with the home leadership and front-line staff to maintainprofessional relationships and conducts themselves accordingly.
- Communicatesabsences, vacations and changes in hours to the home contact person as aprofessional courtesy and in order to ensure continuity of resident care.
- Utilizestherapeutic communication by establishing a professional workingrelationship with respect to professional boundaries, privacy and consentwith clients, residents, Substitute decision makers, PG&T;, coworkersand others within the circle of care for the purpose of meeting theresident’s needs.
- Committedto performance improvement activities including PDSAs, tests of change,working groups, ongoing education, staff meetings and training.
- Provideson site leadership and direction to the BSO Clinical Support Worker in theassessment, application, documentation and demonstration of carestrategies, interventions and recommendations.
- Participatesand supports successful resident transitions across health care sectorswith the resident’s/SDM consent to ensure assessments, care planstrategies and recommendations for care are shared.
- Completeother duties as assigned by the Sub Region RN Manager/ designate.

QUALIFICATIONS

- Currentregistration in good standing from the College of Nurses of Ontario as aRegistered Practical Nurse required
- Minimum of 2 years LTC experience required.
- Soundknowledge and application of practices as they relate to persons withDementia.
- Experiencein Mental Health or related practices preferred.
- ValidG Drivers Licence and reliable vehicle, as travel within your designatedregion is required
