Nurse Practitioner - Temporary - up to 37.5 hours/week
Temporary Positions – up to 37.5 hours perweek (schedule to be established with successful candidate(s)) Hourly rate: $58.30- $64.35
We are seeking dedicated and compassionate Nurse Practitioners(NP’s) to join our team on a temporary basis, providing primary care topatients who do not have access to care and those who are or are at risk ofexperiencing homelessness. The NPs will offer comprehensive care, includinghealth assessments, diagnosis, and treatment, while addressing the socialdeterminants of health. The ideal candidate will have a passion for workingwith underserved populations and ensuring equitable access to healthcare.
Primary Responsibilities
- Provide primarycare services to unattached and/or vulnerable patients, including healthassessments, diagnosis, treatment, and follow-up care.
- Develop andimplement patient care plans, ensuring a holistic approach to address physical,mental, and social needs.
- Collaborate withan interdisciplinary team, including physicians, social workers, and communityhealth professionals, to provide integrated and comprehensive care.
- Engage in healthpromotion and disease prevention activities tailored to the needs of vulnerable populations.
- Advocate forpatients by connecting them with appropriate community resources and support services.
- Monitor andevaluate patient progress, adjusting care plans as needed to achieve optimalhealth outcomes.
- Participates inprogram development aimed at improving healthcare access and reducing barriersfor unattached and vulnerable individuals.
- Participate inongoing quality monitoring and improvement.
Education, Qualifications andSkills
- Currentregistration in positive standing with the College of Nurses of Ontario as a NursePractitioner (Primary Health Care).
- Minimum of 3-5years of clinical experience, preferably in primary care or working with vulnerable populations.
- Strong knowledgeand proficiency in chronic disease management, mental health, addictionservices, and evidence-based practices in primary care.
- Excellent healthassessment, clinical, analytical, and problem-solving skills with a strongcommitment to patient-centered advocacy.
- Ability to workboth independently and as an integral member of a multidisciplinary team.
- Proficient inusing computers and information technology, including electronic medicalrecords (EMR); experience with Practice Solutions is preferred.
- Outstandingcommunication and interpersonal skills, with leadership, organizational, andtime management capabilities.
- Ability toprioritize and manage time effectively in a fast-paced, dynamic environment.
- Fluency in French(spoken and written) is an asset.
Additional Job Requirements
- Must adhere toall TAFHT Policies and Procedures.
- Must be legallyentitled to work in Canada.
- This position isin Timmins ON, the position requires working in office and on site.
- As a condition ofemployment, you are required to submit proof of COVID-19 vaccination and immunizations.
Theabove responsibilities are not to be considered all inclusive; and may beassigned other related duties in the interest of efficient operations of theFamily Health Team.
Thisposition reports directly to the Clinical Director. The location of thisposition could be the Administration, Algonquin West, 101 Mall, Algonquin East,Third Avenue site or any other location within Timmins. The employer reservesthe right to modify the location/site. This position is a bargaining unitposition, represented by OPSEU/SEFPO.
