Key Responsibilities:
• Provide individualized support and wellness planning for clients
• Assist in maintaining clients' housing stability and community engagement
• Facilitate access to resources and financial management help
• Advocate for clients’ rights and needs within the community
• Collaborate with multidisciplinary teams for effective care
Requirements:
• Degree or diploma in health or social services
• Current registration with OCSWSSW
• 4+ years experience in community organizations
• Knowledge of the Ontario Mental Health Act
• Valid driver’s license and insurance covering client transport
