Status: Full Time Temporary, Part Time Temporary, Relief/Casual (Funding Dependent)
Location: Oakville primarily, other Agency locations in the Peel region as required.

About the role:
The Neurorehabilitation Facilitator provides adaptable, client-centered support to adults living with acquired brain injuries (ABI) across a range of settings, including assisted living, day services, and community programs. Working under the direction of Program Managers and clinical teams, this role is responsible for implementing individualized service plans (ISPs), facilitating therapeutic and skill-building groups, assisting with activities of daily living (ADLs), and applying evidence-based neurorehabilitation strategies. Through behavioral support, crisis intervention, and interdisciplinary collaboration, the Facilitator empowers clients to maximize their functional independence, promote community integration, and achieve the highest possible quality of life.
Duties and Responsibilities:
- Individualized Neurorehabilitation & Care Planning: Execute Individualized Service Plans (ISPs) and specialized neurorehabilitation strategies tailored to address cognitive changes, physical limitations, and health conditions following clinical best practices.
- Personal Support Work & Daily Living Assistance: Deliver hands-on personal support work by assisting clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), including personal care, hygiene, mobility, meal preparation, nutrition, household maintenance, and basic budgeting—to promote maximum independence.
- Medication Administration & Safety: Safely administer medications per agency policies and guidelines, ensuring accurate documentation on Medication Administration Records (MARs) and adherence to clinical procedures.
- Behavioral Management & De-escalation: Apply evidence-based behavioral management techniques and behavior support plans, provide behavioral guidance and ABI education to Long-Term Care (LTC) staff, and utilize de-escalation protocols during challenging situations.
- Therapeutic & Peer Group Facilitation: Design, lead, and co-facilitate individual and group therapeutic sessions (such as social skills training, anger management, coping strategies, art, music, and exercise) adapted to participants' abilities and rehabilitation goals.
- 24/7 Assisted Living & Residential Support: Provide 24-hour care within residential and assisted living settings, maintaining a safe, home-like environment centered on rehabilitation, client dignity, and safety.
- Community Integration & Transportation: Facilitate client integration into the community by supporting and supervising clients during outings, assisting with transportation, and helping generalize functional skills in real-world settings.
- Crisis Intervention & Harm Reduction: Address crises, emotional distress, isolation, and mental health or substance use challenges using trauma-informed care, de-escalation techniques, and harm-reduction strategies.
- Progress Monitoring & Clinical Documentation: Observe, track, and document client progress, cognitive and functional status, emotional well-being, skill acquisition, daily logs, and incident reports in a timely, accurate manner.
- Multidisciplinary & Caregiver Collaboration: Work closely with Program Managers, interdisciplinary clinical teams, family members, caregivers, and external service providers to ensure seamless, holistic care coordination.
- Healthcare Navigation & Resource Access: Help clients navigate external healthcare systems, addiction services, mental health resources, and community supports to ensure comprehensive long-term care.
- Environmental Safety & Program Setup: Maintain a safe, therapeutic, and clean environment by assisting with activity setup and teardown, adhering to health and safety standards, and mitigating environmental risks.
Qualifications and Requirements:
- Relevant Education: Diploma or degree in a related health or social services field (e.g., Social Service Worker, Kinesiology, Recreation Therapy, Psychology, Neuroscience, Health Sciences, OT/PT Assistant, or Addictions and Mental Health).
- Clinical & ABI Experience: Proven experience working with adults living with acquired brain injuries (ABI), cognitive impairments, physical disabilities, mental health challenges, or co-existing diagnoses in a rehabilitation setting.
- Mandatory Certifications: Valid First Aid and CPR certifications, along with required annual certification in Safe Management (or willingness to complete upon hire).
- Driver’s License & Mobility: Valid driver’s license and access to a reliable, insured personal vehicle for client transportation and movement between program locations.
- Neurorehabilitation Knowledge: In-depth understanding of ABI principles, aging, cognitive/physical impairments, and evidence-based restorative and compensatory rehabilitation strategies.
- Harm Reduction & Mental Health Expertise:
Working knowledge of trauma-informed care, recovery-oriented practices, harm reduction, substance use support, and crisis intervention protocols.
- Crisis Management & Interpersonal Skills: Demonstrated ability to handle complex behaviors and crisis situations using de-escalation techniques, combined with strong communication skills rooted in patience, empathy, and professional boundaries.
- Physical Capability: Ability to meet the physical demands of the role, including standing/walking for long periods, setting up activity areas, and executing safe client lifts and transfers using proper body mechanics and assistive devices.
- Systems & Community Knowledge: Familiarity with supported independent living, community healthcare resources, and Long-Term Care (LTC) environments and relevant legislation.
- Preferred Specialized Training: Preferred certification in Applied Suicide Intervention Skills Training (ASIST) and completed training in Medication Administration.

- Client-Centred Neurorehabilitation: Deep understanding of acquired brain injury (ABI) principles, applying evidence-based restorative and compensatory strategies to deliver individualized, goal-oriented care.
- Program Facilitation & Adaptability: Skilled at designing, leading, and adapting engaging group and individual sessions that foster skill acquisition, meaningful participation, and client dignity across diverse settings.
- Interdisciplinary Collaboration & Communication: Collaborates effectively with multidisciplinary care teams, families, and external partners while consistently maintaining accurate, timely, and compliant clinical documentation.
Hours of Work
- Full Time: 40 hours/Week ( Day, Afternoon, Overnight)
- Part time: 16 hours/week (Weekends, Day, Afternoon, Overnight)
- Relief/Casual: A minimum of 24 hours/month
- Flexibility required to meet operational, client, and program needs. Evenings and weekends, as needed.

Pay: $24.93-$29.41 per hour
