JOB POSTING -Internal/ExternalPosition: Social Worker – Full Time 35 hours weekSalary: $48 per hourStart: ImmediatelyClosing:August 17, 2026Location: 22 Vaughan Road – Auduzhe Mino NesewinongBackgroundAuduzhe Mino Nesewinong (Place of Healthy Breathing) is an Indigenous‐led primary health care initiative that has supported First Nations, Inuit and Métis (FNIM) community members for more than 5 years. Grounded in Indigenous values, self‐determination, and relationship‐based care, Auduzhe was established to address systemic barriers experienced by FNIM peoples. Through trusted community partnerships and outreach‐based service delivery, Auduzhe has grown into an interdisciplinary primary health care model that provides culturally secure, trauma‐informed, and wholistic care across the continuum, including health promotion, disease prevention, mental health and wellness, and primary care services. Embedded within Na‐Me‐Res (Native Men's Residence), Auduzhe also supports Indigenous men residing at the shelter by delivering culturally safe, trauma‐informed primary health care and mental wellness services in a familiar and trusted setting. Auduzhe's approach prioritizes community‐identified needs, continuity of care, and coordination with Indigenous and allied service providers, while advancing Indigenous ways of knowing and being within primary health care settings.Roles and Responsibilities:Provide culturally safe care to First Nations, Inuit, and Métis community members in TorontoWork independently and as part of a team while supporting clients who present with a range of complex needs such as mental health, addiction, and psychosocial issuesAdvocate for clients and assist in systems navigation including legal, medical and financial assistance, housing, education & employment, health & well-being, transportation, and other servicesAssist in locating culturally appropriate community resources by liaising with community groups and other health/social service agencies to provide referral information for clientsProvide short term crisis prevention/response, de‐escalation, and stabilization through instrumental support and referrals to culturally appropriate crisis servicesFacilitate transportation and/or accompany clients to medical appointments, court, community programs and other appointments as neededEstablish trusting relationships with FNIM community members and familiesCollaborate with psychiatrist, psychotherapist, Community Health Worker,



and medical team within Auduzhe Mino Nesewinong to ensure clients are integrated into the appropriate servicesMeets regularly with individual clients to monitor needs and progress on an ongoing basis, provide case management, referrals, mentoring and coachingTriage and support incoming requests for services and support team as required within the scope of the rolePromote Auduzhe to external neighbourhood agencies and build strong working relationships with community agenciesProvide supports to assist with housing stabilizationProvides intensive case management to individuals experiencing serious mental illness and dealing with possible addictions issues, involvement in the legal system and/or homelessnessAccurate record keeping and timely documentation including services provided/offered and referralsMaintain contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on clients' overall progress while maintaining confidentialityEducate and assist clients regarding rules related to maintaining subsidized housing and completion of yearly eligibility formsOther tasks as required/assigned by Auduzhe leadershipQualificationsA Bachelor of Social Work degree or equivalent combination of education and experienceMember in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)A minimum of 3-5 years' experience in community-based health care is an assetStrong knowledge of available community programs/resources and ability to conduct appropriate client referralsStrong understanding of systems that cause homelessness, poverty and discrimination, and respect for those realitiesDemonstrated respect for, and expertise working with people who are or have been homeless, living in poverty, experiencing varying degrees of mental health, substance use, trauma, social isolation,



unemployment and/or history with the criminal justice systemStrong understanding and commitment to social justice issues and anti‐oppression practicesWell‐developed conflict resolution and negotiation skills, and crisis management skillsAble to work under pressure, handle emergencies with professionalism and defuse crisis situationsHighly developed understanding of and proven ability to communicate appropriate professional boundariesHighly developed ability to prioritize and work through short‐and‐long‐term goals and remain flexibleStrong case management skills are an assetExperience with housing referrals is an assetAnalytical skills to identify and resolve problems, assess client needs, formulate recommendations using sound judgement, determine and take action to address the needs of the clientExcellent interpersonal skills to build trusting relationships, liaise with community professionals and colleaguesExcellent knowledge of and ability to apply harm reduction principles and practices in work with the communityExperience with trauma‐informed care; able to communicate with clients in a culturally safe, empathetic, and professional mannerSolid understanding of the issues facing Indigenous communities and systemic barriers to accessing culturally responsive health careExperience with Indigenous community‐based service; experience in dealing with or knowledge of Indigenous healthAbility to work independently and as part of a multi‐disciplinary teamHighly developed written and oral communication skillsStrong documentation and file management skillsProven ability to give and receive constructive feedbackProficiency in the use of computer technology and using an Electronic Medical Record (EMR)Strong organizational skills and attention to detailAbility to manage multiple tasks in a timely manner and flexibility to adapt to changing workloadProfessional liability insurance coverageProof of COVID‐19 vaccinationNa‐Me‐Res encourages First Nation, Inuit, and Métis applicants to apply and please identify in their cover letter with connections to their community.
