Key Responsibilities:
• Perform initial and ongoing client assessments
• Develop personalized case management plans with clients
• Educate clients in individual and group settings
• Liaise with interdisciplinary services when necessary
• Provide recovery-focused, trauma-informed care
Requirements:
• Baccalaureate Degree or Diploma in Nursing
• 1-3 years of nursing experience required
• Eligible for registration with College of Nurses in PEI
• Knowledge of substance use and Mental Health Act
• Experience with Telus EMR is preferred
