Grey County's Long-Term Care department is recruiting a permanent, full-time RAI Coordinator at Rockwood Terrace in Durham, ON (Municipality of West Grey). This job posting is for an existing vacancy.

Purpose

Reporting to the Director of Care, the RAI Coordinator is accountable for the Resident Assessment Instrument – Minimum Data Set 2.0 (RAI-MDS) program to support the delivery of optimal care and service, based on legislative requirements and organization standards.

The RAI Coordinator is required to perform their duties in a manner that demonstrates a commitment to quality care in accordance with the organizations vision and mission.

Responsibilities

- Acts as the RAI-MDS resource for all participating team members in the home
- Manages the coordination and completion of all components of the program
- Establish, maintain and communicate the current RAI-MDS schedule to the inter-professional team
- Establish an effective process for auditing to ensure RAI-MDS accuracy and compliance with coding, resident assessment protocols (RAPs) and care plan updating
- Complete a monthly review and analysis of each resident’s RAI-MDS to ensure accuracy of assessments and meeting of resident needs and that documentation reflects resident needs
- Monitor and analyze RAI-MDS outputs and report and communicate with the management team and support services committees
- Reinforce expectations of the RAI-MDS program as assigned by management and escalate any unresolved concerns to the direct supervisor
- Ensure all RAI-MDS documentation is completed by the inter-professional team by the deadline and report any conflicts to the Director of Care or designate
- Produce monthly reports by the due date and distribute to team members to assist in clinical and operational decision-making
- Monitor and conduct activities within the RAI-MDS budget
- Maintain competency by attending/participating in educational sessions (i.e.



Canadian Institute for Health Information (CIHI) education portal and annual Assessment Intelligence Systems (AIS) renewal)
- Adhere to all RAI MDS 2.0 outlined time frames
- Sign off on records prior to submission to CIHI
- Assume responsibility for the computerized Data Submission process and maintain records following each submission within the time frames outlined by the Ministry of Long-Term Care (MLTC)
- Track rejections and errors during submission process to CIHI and engage in corrections and re-submissions as per MLTC deadlines
- Work in consultation with the Leadership Team and the Support Services Consultant team to ensure data integrity, data utilization and interpretation
- Follow all support services policies and procedures and contribute to the development of RAI-MDS specific policies
- Monitor and analyze RAI-MDS outputs and report and communicate with the Leadership Team and applicable committees
- Provide RAI-MDS and all electronic documentation training, initial and ongoing to the inter-professional team
- Lead AIS related activities including
- Manage accounts for team members in AIS Central
- Support team members learning needs in using AIS Mastering the RAI
- Ensure team member complete their required AIS Proficiency Evaluations in a timely manner and provide support where needed
- Run reports in AIS to manage progress with Ministry mandated requirement for AIS Proficiency Evaluations
- Identify RAI-MDS 2.0 areas that team members have good knowledge for coding and areas where improvement is needed
- Work in accordance with the organizations Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act
- Perform other duties as required

Working Conditions
- Modern facility, temperature controlled, smoke free setting with a combination of natural and fluorescent lighting
- Engagement and/or exposure to various therapy programs not limited to pet therapy, spiritual events and musical therapy
- Required to meet deadlines

Knowledge and Skill

- Current general certificate of registration with a college of a regulated health profession or Ontario College of Social Workers with a minimum of 3 years clinical, project experience
- Current AIS Certification
- Successful annual completion of RAI MDS certification per Ministry of Health requirements for AIS
- RAI-MDS education is an asset
- Current license to practice
- Must possess a minimum of three years clinical experience
- Familiarity with RAI-MDS 2.0 preferred
- Previous experience in adult education strategies/facilitation
- Problem solving, and analytical skills are required
- Must possess excellent assessment skills
- Must be proficient in Microsoft Office Suite, internet use and computerized documentation
- Effective verbal and written English communication skills required
