Responsibilities

- Deliver comprehensive patient care by assessing needs, developing care plans, and implementing treatments aligned with medical protocols.
- Administer medications and treatments accurately, following prescribed orders and safety guidelines.
- Utilize electronic health record systems such as Epic and Cerner to document patient information, treatment plans, and progress notes efficiently.
- Collaborate with multidisciplinary teams to coordinate case management efforts, ensuring seamless patient transitions across care settings.
- Conduct specialized care for pediatric and senior populations, including dementia and hospice care, with sensitivity and professionalism.
- Perform phlebotomy procedures and laboratory specimen collection with precision and adherence to safety standards.
- Educate patients and families on health conditions, medication management, pain control strategies,



and post-discharge care instructions.

Requirements

- Valid RN license in the state of practice with current certification.
- Proven hospital or acute care experience demonstrating proficiency in patient assessment and emergency response.
- Familiarity with electronic health records such as Epic and Cerner systems.
- Experience working with diverse patient populations including pediatrics, elderly, or those requiring dementia or senior care.
- Strong knowledge of medical terminology, medication administration protocols, and pain management techniques.
- Previous experience in home care or assisted living environments is highly desirable.
- Ability to perform phlebotomy and laboratory procedures accurately while maintaining infection control standards.
