In this full time permanent role, you will manage the primary care connection clinic, ensuring efficient processing and prioritization of client intakes. Your expertise in triage assessment will guide clients through their health service needs, while effectively liaising with various healthcare providers and community partners.
Key Responsibilities:
• Oversee clinic intakes and internal referrals
• Conduct comprehensive intake triage assessments
• Maintain up-to-date knowledge of resources




• Ensure accurate and complete intake documentation
• Facilitate communication with primary care providers
Requirements:
• Registered Nurse or Registered Practical Nurse designation
• Minimum two years' relevant experience in primary care
• Strong organizational and decision-making skills
• Experience with marginalized populations is an asset
• Competency in MS Office and electronic medical records
