Fraser Health is seeking Casual Social Workers to join our Long Term Care team. In this rewarding position, you will assess clients’ emotional and social needs while offering clinical counseling and facilitating community resource links. Your expertise will contribute to enhancing the lives of seniors experiencing transitions due to illness and disability.

Key Responsibilities:
• Conduct psycho-social assessments for clients and families
• Deliver clinical counseling services regarding adjustments to care
• Consult on high-risk factors and educate families




• Coordinate admissions and provide family orientation
• Contribute to ongoing care planning and development

Requirements:
• Bachelor's Degree in Social Work from an accredited program
• At least one year of related experience preferred
• Current registration with the BC College of Social Workers
• Familiarity with community resources and legislation
• Proven ability to work within interdisciplinary teams
