This part-time position requires 1 weekday shift and a weekend rotation every three weeks, providing vital care in community settings. Key Responsibilities:
• Provide in-home clinical care including wound care and IV therapy
• Collaborate with multidisciplinary teams for patient-centered solutions




• Mentor and guide patients and families through healthcare options
• Administer medications and conduct health education
• Facilitate coordination of care services for clients

Requirements:
• Registered with the College of Nurses of Ontario
• Valid driver’s license and insured vehicle required
• Clear Criminal Record Check needed
• Strong communication skills in English; French is an asset
• Ability to work versatile shifts including weekends
