Traverse Independence was founded in 1975 as the Participation House Project Waterloo Region, an agency dedicated to developing housing with support services for people with physical disabilities.

Since then, our organization has evolved from offering services for adults with physical disabilities to serving those with a brain injury in the Region of Waterloo and County of Wellington, across eight different programs.

One of these exceptional services is our outreach program which offers both a Mainstream program (in the person''s home) and ABI on the Streets which works with people who are experiencing homelessness.A DAY IN THE LIFE OF A COMMUNITY FACILITATORAs a Community Facilitator you will work in a low barrier program, building trusted relationships with your clients, which in turn will support them to access the services they need when experiencing homelessness, living rough or living in safe and appropriate housing.

You will provide support across the Region of Waterloo,



and in partnership with the client/caregivers will develop goals, track outcomes, achievements and support the client as they move forward.

As a Community Facilitator, you will demonstrate the ability to work with clients who exhibit challenges with mental health, pehaps addictions, insight, skills and other symptoms of brain injury.

Additionally, you will be required to be a liaisonand work directly with professionals, community resources, and other significant support persons who are involved with a client as part of an integrated community team.

You will report to a Service Coordinator and have access to regulated staff such as ABI specialized psychiatrists, an Occupational Therapist, Behaviour Therapist and an Intensive Case CoordinatorMUNITY FACILITATOR REQUIREMENTS:Candidates must have proven organizational skills, interpersonal skills, communication skills (both written an oral), and the ability to work as a member of a multi-disciplinary team with a comprehensive understanding of Microsoft Office products.

Experience working with people with Acquired Brain Injuries is a benefit, along with mental health and addictions but not necessary as we offer a comprehensive training program both internally and through the Ontario Brain Injury Association.





This is a great chance to add an ABI specialization to your skillset.

Education: Community College Diploma in Social Services, Developmental Services, Health Care, or related discipline.

Knowledge/Skills: Demonstrated experience in behaviour management skills is of benefit.

Experience with people with disabilities such as mental health and/or addictions is a great asset but not necessary.

Strong utilization and understanding of M365 - Teams/Share

Point.

Demonstrated organizational skills, interpersonal skills, and communication skills.

Ability to work as a member of a multidisciplinary team.
