The Care Company is seeking dedicated Community Nurses to provide exceptional care for ALC clients. In this full time position, you will conduct comprehensive assessments, develop personalized care plans, and collaborate with care teams. Key Responsibilities:
• Conduct detailed client assessments for individualized care plans
• Administer medications and monitor vital signs accurately
• Collaborate with care teams for comprehensive client support




• Provide in-home mobility assistance and physical therapy exercises
• Maintain accurate documentation in compliance with standards

Requirements:
• Active RN registration with the College of Nurses of Ontario
• Current CPR and First Aid certification
