TCRHCC seeks a dedicated Certified Nursing Assistant to join their team, focusing on diabetes and preventive care. This role involves performing vital sign assessments, foot exams, and coordinating patient education under registered dietitian supervision. Ideal candidates should have a robust background in patient care and a commitment to high ethical standards to enhance the wellness of patients.
Key Responsibilities:
• Welcome and greet patients and families in-person and on the phone
• Conduct blood glucose and BMI screenings for patients
• Educate patients and families on diabetes care and nutrition
• Document patient care to ensure reimbursement maximization
• Initiate referrals for additional healthcare services as needed
Requirements:
• High school diploma or GED required
• Current Nursing Assistant certification/license in any state
• Minimum one year of direct patient care experience
• Basic Life Support certification within six months of hire
• Proven ability to handle stress and maintain confidentiality
