Job Description: Reporting to the Care Team Manager the Resident Care Coordinator is responsible for supporting the highest standard of care and service provision to residents living on a given neighborhood. As an experienced practitioner, the resident care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of resident care, and provide support and teaching to the resident care team (including residents and residents families) within a cooperative environment.



The routine procedures are generally predetermined but critical thinking and independent judgment is an integral part of the position.
Human Resources Exemption: No
Experience Minimum of three (3) years relevant long term care nursing experience
Education Bachelor of Science in Nursing (BScN)
Licenses Basic Life Support Certificate (BLS)
Other Education and Training Psychiatric Nursing diploma is acceptable
Nursing diploma is acceptable
Bachelor of Psychiatric Nursing is acceptable
Competencies Demonstrates leadership skills
Knowledge, Skills & Abilities Knowledge of practical application of a broad range of clinical intervientions
Demonstrates ability to work as a team member
Demonstrates effective verbal and written communication skills.
Knowledge of long-term care needs of adults and their families
