(T/26-99) Personal Support Worker - Rehabilitation (Temporary Part time) Responsibleand accountable to the Manager of Rehabilitation and Pharmacy Services toassist patients with personal care and activities of daily living in theassigned units by following the individualized care plans.
Key Accountabilities Performs all duties in accordance with the MissionStatement and policies and procedures of St. Joseph’s Health Centre asadopted from the Ministry of Health guidelines.
The Helping Role Preserving the personhood of patients
Encountering patients in a caring way
Maximizing the individual’s participation and control
Providing comfort and facilitating communication
Encouraging and supporting individuals’ families to participate in care

The Provision of Patient Care Assisting with personal hygiene (bathing, dressing, hair care, mouth care, nail care,and shaving)
Assisting with nutrition (providing fluids and nourishments as required, meal set up, assistance with feeding, reporting of intake, and taking weights asrequired.)
Assisting with urinary and bowel elimination (toileting, disposable briefs, emptying ofurinary and colostomy drainage bags, documentation and reporting of output)
Promoting comfort by assisting with positioning, mobility, and ambulation as per careplan (positioning and turning of patients ; using mechanical liftsappropriately; and reporting of any condition or mobility changes)
Promoting skin integrity (providing good skin care through washing, positioning, and turning of patients; ensuring that environment is safe of obstacles toprevent injuries; completing basic wound care dressings; and reporting any changes in skin integrity). Immediately correcting if possible and reportingany situations that can or will effect the health and safety of the patients.
Assisting with exercises




Facilitating appropriate rest and socialization
Accompanying patients who require an escort, to appointments
Recording documentation in an accurate and timely manner according to policies and procedures

Establishing and Maintaining Healthy Interpersonal Relationships Developing and preserving therapeutic PSW /SSW client relationships
Developing and preserving team-oriented relationships among the health care team and other programs/departments

Ensuring the Quality of Health Care Practices Contributing to the quality of working life of peers and colleagues
Improving the quality of life of patients through a patient and family centred careapproach
Learning and using new concepts and approaches in the care of adults
Participating in improving the care provided to adults
Consulting with peers and other health providers in approaches in the care of adults
Contributing to and utilizing the established plan of care for patients

Maintaining Professionalism and Work Role Competencies Attending and participating in educational in-services
Demonstrating professionalism
Providing services in a caring, respectful, and cost effective manner
Participating in research and continuous quality improvement activities for improving the care provided to adults

Qualifications & Experience Personal support worker certificate or diploma from Ministry of Health of Ontario, Long Term Care Division approved educational facility




Experience in long term care or hospital setting preferred
Demonstrated written and verbal communication skills, and excellent interpersonal skills
Solid organizational skills and the ability to prioritize required.
Ability to function as a team member in a collaborative environment required.
Ability to accept direction and work independently required.
Knowledge of general persuasive approach preferred.
