We are looking for two dedicated Nurse Practitioners committed to providing comprehensive care for patients at risk of homelessness.
Your experience with chronic disease management and mental health will play a vital role in addressing social determinants of health. Collaboration with an interdisciplinary team will ensure integrated care for those in need. Key Responsibilities:
- Provide primary care and follow-up for vulnerable patients
- Develop individualized patient care plans
- Collaborate with social workers and healthcare professionals
- Engage in health promotion and disease prevention
- Advocate by connecting patients to community resources
Requirements:
- Current NP registration with the College of Nurses of Ontario
- Minimum 2 years of direct clinical experience
- Solid skills in chronic disease management
- Proficiency in electronic medical records
- Fluency in French is an asset
