We are seeking twodedicated and compassionate Nurse Practitioners (NP’s) to join our team,providing primary care to patients who do not have access to care and those whoare or are at risk of experiencing homelessness. The NPs will offercomprehensive care, including health assessments, diagnosis, and treatment,while addressing the social determinants of health. The ideal candidate willhave a passion for working with underserved populations and ensuring equitableaccess to healthcare.

Program Eligibility Allowances:
- CCPNgrant of $25,000.
- Education grant of $1,500.
- NorthernTop-Up grant of $10,000.
- NorthernRelocation grant of $10,000.
ProgramEligibility Criteria:
- Not have been employedas a nurse in Ontario in the six months prior to being hired.
- Holda Certificate of Registration in good standing from the College of Nurses of Ontario.
- Begin their 2-yearcommitment between April 1, 2026, and March 31, 2027.
- Commit to two-years ofemployment at a full-time work schedule offered by the employer.
- Not simultaneouslyreceive funds from the Ministry of Health for the Tuition Support Program for Nurses.
- Not be hired throughthe Nursing Graduate Guarantee program.
- 2-yearcommitment required to be eligible incentives and grants.
PrimaryResponsibilities
- Provide primary careservices to unattached and/or vulnerable patients, including healthassessments, diagnosis, treatment, and follow-up care.
- Develop and implementpatient care plans, ensuring a holistic approach to address physical,mental, and social needs.
- Collaborate with aninterdisciplinary team, including physicians, social workers, andcommunity health professionals, to provide integrated and comprehensivecare.
- Engage in healthpromotion and disease prevention activities tailored to the needs ofvulnerable populations.
- Advocate for patients byconnecting them with appropriate community resources and support services.
- Monitor and evaluatepatient progress, adjusting care plans as needed to achieve optimal healthoutcomes.
- Participates in programdevelopment aimed at improving healthcare access and reducing barriers forunattached and vulnerable individuals.
- Participate in ongoingquality monitoring and improvement.
Education,Qualifications and Skills
- Current registration in goodstanding with the College of Nurses of Ontario as a Nurse Practitioner(Primary Health Care).
- Strong knowledge andproficiency in chronic disease management, mental health,
addictionservices, and the application of evidence-based practices in primary care.
- Excellent healthassessment, clinical, analytical, and problem-solving skills with a strongcommitment to patient-centered care and advocacy.
- Ability to workindependently and effectively as an integral member of a multidisciplinaryteam, balancing both autonomous decision making and team collaboration.
- Proficient in using informationtechnology and electronic medical records (EMR); with experience inPractice Solutions preferred.
- Must possess a minimum of 2years of direct clinical experience as a Nurse Practitioner, acquired in healthcarethat requires independent practice. This experience must demonstrate theability to independently assess, diagnose, treat, and manage patient careacross a broad range of clinical conditions.
- Experiencemust include working independently and making clinical decisions withoutdirect supervision, ensuring continuity of care, particularly for complexcases and high-acuity patients.
- Aproven track record of independently managing patient care plans, makingdiagnostic and therapeutic decisions, and providing advanced clinicalexpertise in primary care delivery.
- Outstandingcommunication and interpersonal skills, with strong leadership,organizational, and time management capabilities.
- Ability to prioritizeand manage time effectively in a fast-paced, dynamic environment.
- Current CPRcertification.
- Fluency in French(spoken and written) is an asset.

The location of this position could be theAdministration, Algonquin West, 101 Mall, Algonquin East, Third Avenue site orany other location within Timmins.
