Nurse Practitioners: two part time positions for evening and weekend coverage. Humber Meadows wishes to have one nurse practitioner on-site during each evening Monday to Friday (from approximately 4pm to 9pm) and for approximately six hours on Saturday and six hours on Sunday. Ideally, each part-time Nurse Practitioner will work some evenings and Saturday or Sunday. The Home is open to discussing this.

Hourly rate is $66.67 CAD.

Reason for Posting: Newly created role

Resident Care & Support In this role, you’ll be at the forefront of clinical care:

- Collaboratively provide care with our interdisciplinary team members, including leading and supporting Registered colleagues in all clinical matters.
- Use your expertise to conduct thorough health assessments and collaborate with physicians on diagnosing acute and chronic conditions.
- Work closely with residents, families, and friends to conduct resident-centered reviews, evaluations and adjustments to care and their individual plans.
- Regularly engages residents in dialogue to determine what is important to them for health and quality of life.
- Order and interpret diagnostic tests, and use this information to make informed clinical decisions.
- Provide data and insight for High Intensity Needs Submissions, shaping care initiatives.
- Collaborate with external parties such as hospitals including Acute Care, LTC Hub, out reach teams, etc.

Operational Excellence

As part of our leadership team, you’ll also:

- Mentor and coach staff, driving quality improvement initiatives and supporting research efforts.




- Foster therapeutic relationships with residents, promoting holistic health and healing.
- Play an active role in representing HMLTCH within the community, contributing to the continuous growth of the Centre.

What We’re Looking For

- Skills & Experience:
- MS Office (Intermediate)
- Electronic Health Documentation (Expert)




- 5+ years of experience in geriatric/long-term care, rehab, medical/surgical, and wound care
- 3+ years of adult/primary care setting as a Nurse Practitioner
- Gerontology, preferred.

- Education:
- Required: Bachelor’s Degree in Nursing
- Current registration with the College of Nurses Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice with no restrictions and in good standing
- Specialist certification in gerontological nursing, preferred
- Post-graduate certificate and experience in palliative care, an asset
- Member of NPAO required
- Member of RNAO preferred.

- Licensing:
- Required: RN Extended (Nurse Practitioner), CPR/First Aid
