About the Role

HealthOPM is seeking compassionate and dedicated Personal Support Workers (PSWs) to join our growing team in Hamilton, ON. In this rewarding community-based, home-to-home role, you'll make a meaningful difference in the lives of seniors by supporting their daily routines, well-being, and independence.

- Location: Hamilton, ON (Community Home-to-Home)

- Shift: 5:00 PM 12:00 AM

- Pay Rate: $20/hour

- Position Type: Part time

- Transportation: Must have a valid drivers license and reliable vehicle

Key Responsibilities

- Assist clients with activities of daily living (ADLs) and personal care

- Provide emotional support and meaningful companionship

- Observe, document, and report changes in health or behaviour

- Support clients with mobility, hygiene, and nutritional needs

- Collaborate with nurses and other healthcare professionals

- Follow individualized care plans and maintain accurate charting

- Adhere to infection prevention, safety,



and confidentiality protocols

Requirements

- Valid PSW Certification (Ontario)

- Recent Vulnerable Sector Check (within 12 months)

- Clear 2-Step TB Test

- Valid drivers license and reliable vehicle

- Strong communication and interpersonal skills

- Experience with dementia care and GPA/NVCI training (preferred)
