Vancouver Coastal Health is seeking a dedicated and skilled Mental Health and Substance Use Case Coordinator Outreach Nurse. In this essential role, you will collaborate with a multidisciplinary team to provide intake, triage, and outreach services for individuals with mental health issues. Key Responsibilities:
• Conduct intake and outreach for mental health services
• Develop, implement, and evaluate care plans

Requirements:
