The Ontario Health Team of Northumberland (OHT-N) seeks an experienced Registered Nurse to serve as the SCOPE Nurse Navigator on a temporary part‑time basis.
Working within the Primary Care Access and Attachment Initiative, the Navigator will support primary care providers by facilitating access to hospital, community, and diagnostic services for patients with complex needs. The role requires current RN registration and solid coordination skills.
