Join our team today!Care Assessment Coordinator (RPN Required)Mission: The Care Assessment Coordinator is responsible for completing clinical assessments for prospective residents in the community, at home and hospital settings. Working closely with Lifestyle Consultants, this role supports new business development by ensuring timely, accurate assessments and helping guide families through the transition to retirement living. This position does not provide in office nursing care.Key OutcomesTimely completion of clinical assessments for prospective residentsIncreased conversions and occupancy through strong LC partnershipPositive experience for families navigating care decisionsKey ResponsibilitiesConduct clinical assessments for prospective residents in hospitals, homes, and the communityPartner closely with Lifestyle Consultants to support the sales process and new resident transitionsProvide same-day or timely assessments to support hospital discharges and urgent inquiriesAct as a clinical resource to help match residents to the appropriate communityBuild and maintain relationships with hospital staff, discharge planners,
and community partnersTravel within the region as required; flexible hours based onbusiness needsMaintain accurate documentation and assessment recordsQualificationsRegistered Practical Nurse (RPN) in good standingExperience in geriatrics, retirement living, or community care preferredStrong assessment, communication, and interpersonal skillsSelf-motivated, organized, and comfortable working independentlyPassion for supporting seniors and their familiesSalary: $70,000 - $80,000Cogir is committed to ensuring that all employment practices are inclusive.
