Vancouver Coastal Health is seeking a Social Worker to join the Mental Health Housing Services team in North Vancouver, BC. Working within a client- and family-centered Recovery model, you’ll collaborate with an interdisciplinary mental health team to support recovery and transitions of care.
You will conduct comprehensive social and psychosocial assessments, provide individual, family and group counselling, and advise on complex discharge and referral needs.
