The Case Manager oversees complex client cases and coordinates services to ensure clients receive comprehensive and effective support. The role involves assessment, planning, monitoring, advocacy, and collaboration with internal and external service providers.
Key Responsibilities
- Conduct comprehensive assessments.
- Develop and implement individualized service plans.
- Coordinate multidisciplinary services and supports.
- Monitor client progress and outcomes.
- Maintain detailed case documentation.
- Participate in team meetings and case reviews.
- Advocate for client needs and access to services.
- Build team-oriented relationships with community agencies.
Qualifications
- Degree in Social Work, Human Services, or related field.
- Experience in case management and service coordination.
- Strong organizational and analytical skills.
- Excellent written and verbal communication abilities.
- Knowledge of Indigenous cultures and community resources.
- Valid Manitoba Driver's Licence.
- Child Abuse Registry, Adult Abuse Registry, and Criminal Record Checks required.
