Key Responsibilities
Deliver direct patient care in diverse healthcare settings, including acute care, assisted living, and medical offices.
Administer medications and treatments as prescribed by healthcare providers.
Monitor and assess patient conditions, focusing on pain management and symptom control.
Collaborate with interdisciplinary teams to create and implement individualized care plans.
Assist in case management and coordinate care for patients with complex medical needs.
Accurately document patient information in electronic health record systems such as Cerner.
Educate and support patients and their families regarding healthcare needs and care plans.
Experience & Qualifications
Minimum of 1 year of experience as a Registered Practical Nurse (RPN).
Experience in acute care, dementia care, hospice care, or other specialized areas is an asset.
Robust skills in medication administration and familiarity with various administration routes.
Knowledge of pain management protocols and best practices.
Familiarity with medical office procedures and healthcare documentation.
Demonstrated ability to provide compassionate, patient-centered care.
Exceptional communication skills for effective interaction with patients, families, and healthcare teams.
Requirements
Current registration with the College of Nurses of Ontario (CNO).
Completion of an RPN certificate program.
