Enhance community health as a Hospital Based Case Coordinator with Winnipeg Regional Health Authority. This in-person role focuses on client assessments and discharge planning to ensure seamless transitions to home care services.
As a key member of the Home Care Program at Seven Oaks General Hospital, this position involves conducting eligibility assessments and developing coordinated care plans with clients and interdisciplinary teams. You will play an essential role in facilitating secure discharges from acute settings while liaising with local health services and family members for optimal client support.
Key Responsibilities:
• Conduct client assessments for home care eligibility
• Develop and coordinate individualized care plans
• Liaise with hospital and community service providers
• Evaluate client needs for potential care home placement
• Provide qualified intervention when necessary
Requirements:
• Minimum three years recent nursing experience
• Graduate of a Registered Nursing education program
• Experience with case coordination preferred
• Competence in Procura© and interRAI-HC© preferred
• Valid registration with CRNM
